Could be any number of things. I'd start with a basic tune up. Plugs, wires, distributer cap and rotor. I had a 4x4 v8 Dakota an got 17mpg on a good day, in my experience that is way too low for a blazer lol. Mine gets up to 22+mpg freeway! Hopefully somebody else will chime in.

Everything Jakeduece said, plus check for leaks in the fuel injection system. Take off the air intake and throttle body. Look inside for fuel pooling and "wash". You can see a lot with a flashlight and small dental-type mirror. If the top of the intake manifold looks too clean in spots, fuel may be leaking from an injector.
Unless you've done it, the throttle body will undoubtedly need cleaning anyway. Use the appropriate spray product. An old toothbrush sometimes helps if there's a lot of junk on it.
I just did a 350 mile trip in my 2002 and averaged 21mpg on highway and secondary roads. Last time I checked, I got 19mpg around town.

Judging from your screen name, a 4wd Blazer will get worse mileage anyway, just from the extra weight and resistance of the front axle and 4x4 equipment. Then, add the extra unsprung weight of a 20" wheel and tire combo, and your mileage is going to suck. Also, the larger wheel/tire size can throw off your odometer, and thus affect your calculated MPG.

I get 16 mpg all day everyday. 98 blazer 4x4. Just put head gaskets on and rebuilt everything from the head's up and still only get 16 mpg, lower engine is in great shape. I've owned this for 8 years and the gas mileage sucks!
All I use this for is shuttling the kid's back and forth to school, just to keep the battery charged.
This Blazer is in great shape, no rust, dents, scratches, interior is dam near perfect!
But at 16 mpg it sit's in the driveway most of the time
My MK8 and Jeep get better mileage
But it's dependable and when I need it.....it's there!!!
