Anything is possible. You will have to modify the rod inside, fab up a mount for the handle on the inside, and mod the inside cover, but it should not be all that difficult. Pop off the cover and have a look at the support inside.

Since I do not have a photographic memory, some pictures of the inside of the 1st gen gate would help me help you.

ok, ill get some pics snapped today ill post them up tonight, my plan is that the tailgate i have now is destroyed, its rotting out on the bottom, i backed into a pole so its all pushed in, theres a crack down the middle that goes to the bracket for the tire carrier and not to mention a hole i can put my finger threw caused by the tire carrier bracket when i backed into that pole. i am gonna get a tailgate at the junkyard next week, im going to mount the license plate in the tailgate, and because the little twist lock broke off i was thinking of just making the handle on the inside, unless they can put the key piece into a new twist lock

You should be able to transfer the lock cylinder over to a new handle.
