11.5 will be fine on the stock rim. Lots of people have run 12.5 on stock wheels with no problems.

I don't know if 2wd ones ever came with hooks or how they were attached.

Yes, the exhaust coming from the drivers side manifold interferes with the driveshaft. You can get a y-pipe from a 2wd or modify the stock one.
