The first/second problem is most likely a vacuum line leak.Vacuum not only controls the AC/heat doors but also activates the 4wd.

With 111,000 miles I would NOT recommend a trans flush but just a fluid/filter change yearly.Many high mileage 4L60Es have been finished off by flushing them.
