Default Vacuum Hose Failing!!! What do I do??

Ok, so I am new here. I wanted to see if anyone had suggestions for replacing all the vacuum hoses in the engine bay... Does anyone have a diagram for where they all go?? Where do YOU buy the vacuum hose? What size hose are all the hoses?
I have on that is so brittle it broke in half.

Also, while I am here (not sure if this is the right tab for this question), I have two check engine codes... What did (or would) YOU do to fix them?

One - P0102, Mass Air Flow (MAF) Sensor Circuit Low Input.
I have cleaned the MAF Sensor, Checked the air filter, and looked for leaks around the hose after the MAF sensor.

Two - P0741, Torque Converter Clutch (TCC) Circuit Performance or Stuck Off.
I have good, clean trans fluid. How likely is it to be the TCC Solenoid?

Thanks in advance for any responses, help, suggestions, recommendations, or comments!

For the vacuum lines, I created these posts with what I learned about the vacuum lines. The first is for trucks with a 3-button transfer case and the second is for the 4-button types.
