I have three questions for you...

Question #1:
How much does a house weigh?

Question #2:
How much weight can a rural two-lane bridge hold and is this home
Now located in a flood zone???

and

Question #3:
Would this be covered by
home insurance, auto insurance,
or do just call roadside assistance?

I would think to fix that, you would need a couple people to lose their jobs, and a big pile of money!

Chances are that house breaking the bridge could have been avoided with spaced axles... using two axles next to each other is just creation of amazing shear.....

As for getting the house out of the situation.... thats a tough one... Because if you dont support both sides, you drop it in the lil creek/lake/water

My guess would be a temporary bridge built under it at a slight angle to ramp it up and back on solid ground. The house will need a bit of attention also. there's a huge buckle in the side now.
