Default Rear end clunk sound

Ok guys I have a 1997 blazer 4.3L 4x4 I have a strange thing going on and it prob happens about half of the time I would or maybe a bit less but when I am at a complete stop and take off it will make a clunk sound and a jerk like feeling and then take off just fine this does not happen every single just about half or so. The tans shift normally so I am tring to figure out if the rear end is getting ready to go out? thanks if you are able to help me out with any ideas.
