that is a unilug wheel and will probably fit. I had a set of unilug 18's and they fit on a multiple of vehichles. If your looking to put this on a blazer you need to look at tires also because i could only fit a 18 with a decent sized tire. without having it rub.

Well from my looking the 5x4.75 is a rather uncommon bolt pattern, and such you would need to see a listing of bolt patterns that this rim is compatible with.

5 on 4.75" is the correct bolt pattern and you have a bunch of options.
Trans Am, Firebirds, Corvette have the same bolt pattern. Keep shopping.

YES!

Don't blindly buy the rims though. GM used a variety of backspacing an rim diameters over the years, even on the same models. Like Firebirds come in 15x7 and 16x8. Check that backspacing. Know what your buying before you buy it, but yes GM has used 5 on 4.75" bolt pattern on a number of vehicles over teh years. Swing by your local tire center and see if you can browse through a catalog. Look in back at the various models and see what is stock for the various years.

I think American Racing has it on line, however its been a while since I went looking for it.
