Default 96 stalls after it gets warm lot of codes.

Straight to the point. Blazer runs great till it gets fully warm. After that very rough idle and will die when put in drive most times or just idle so poor that the engine will shake violently. Does not do it in reverse.
The cat conver.. went out a while back just replaced that yesterday.
So put the scanner on it and at first it said p0339, p0341, p0302
Checked the wire to cyl2 and cleaned the boot. cleared codes and then after a complete warm up died and would not start
threw the following.
p0108 map/baro circuit high input
p0339 crankshaft position sensor sensor a ckt intermittent
p0341 cam position sensor a-bank1 ckt/range perf
p1361 manufacturer control - ignition system or misfire
----------------
I/M monitor
misfire mon, fuelsys mon, comp component, all said OK. cat mon, evap sys, oxy sens mon, oxy sens htr, egr sys mon all were inc. Every thing else in I/M monitor came up N/A.
-----------------
freeze data
pid 190 of 192
dtc that caused freeze frame
p0339
engine rpm 6368
load value 6.2%
airflow rate 13.11 gr/sec
map sensor 85kpa
coolant temp 186
short term fuel trim1 0.0%
long term fuel trim 1 0.0%
stft2 0.0%
ltft2 0.0%
stft3 50.0%
ltft3 -38.3%
ltft4 -45.2%
stft4 57.0%
vehicle speed 0 mph
fuelsystem1 open
fuelsystem2 open
--------------
Then I started it a few times and checked codes again and got a
p0118 Engine coolant temp circuit - high input.( DID THAT WITH OUT CLEARING CODES)
----------------
There is the long list of info the scanner gave me.
She starts and runs like a champ till warm. Then stalls out. and won't start till it cools back down. Sure could use some help.

Thanks for the reply. Is there a way to test that?

Tap the +5V reference wire at any one of the sensors showing the code and monitor it. It should stay at +5V.
