Default bad coolant leak

So today I was driving to my girlfriends house. And I noticed my temp was a little high. I didn't think anything of it due to the fact it was pretty hot outside and I had the a/c turned up. So I kept driving. But as I pulled up to her front gate I rolled down the window to put in the gate code and could smell coolant, I looked down at my temp and it was very high. So I drove to her house and I pulled into the driveway and smoke/steam started pouring from the hood and there was a trail of coolant all the way upto where I parked and it was still coming from under the car. I popped the hood and it seems like the leak is coming from somewhere near the back of the motor. Does anyone know what could be the problem? Any help would be great.

You'll have to be the one to find the exact source of the leak. It could be the intake manifold gasket, a heater hose, a blown out casting plug (freeze plug), etc.

Its at the shop right now I just wanted to know what it could be. I thought it was kinda weird. Thanks for the help. The motor only has 5k miles on it. Would a freeze plug fail that fast?
