Default 1987 Chevy Blazer Clutch/Transmission Issue

My question is about a 1987 Chevy Blazer. The slave cylinder and the master cylinder have been replaced. The clutch wasn't building any pressure. We finally got the pressure built but it slipped within less than a block of driving.

It appears that the throw out lever (or maybe it is called the clutch fork??) is slipping off the throw out bearing. Does anyone know why this might be happening?

Is the clutch fork still fixed on the ball/socket joint on the bell housing. If it is, then you may have a damaged fork where it rides on the throw out bearing. That and/or maybe a broken throw out bearing.
