First off you cannot "gain hp" from an exhaust systtem. The hp comes from the engine, the exhaust system restricts the amount of hp the engine can create.
The simple answer to your question. 2 1/2" pipe.
The muffler with the best flow rate is a glasspack.
Most aftermarket performance mufflers have baffles that redirect the air flow to aquire a tone. They provide less restriction compared to a stock muffler, and thus increase your engines hp slightly, less then 5hp. A stock muffler runs the air through the muffler 3 times before it exits the muffler.
The best investment, if you really want to increase hp, is to get rid of the stock exhaust manifolds in favor of some headers. The exhaust manifold/headers, collector pipes, y-pipes is where you will have the most effect on your engiens performance. By time the exhaust gets back to your muffler, there isn't much difference you can make in freeing up engine hp.
The cat back system is designed for sound. Is that tone really worth $300.

Here is a comparison pic I shoped together to show the differnce between the 2 models of gibson exhaust for the 2dr that you refer to. As you can see the ZR2 only version (Bottom) looks almost identical to the standard non zr2 (Middle) version. So I would consider sending them an e-mail asking them if the 4dr model (Top)shown will work for your 03...

I have a Dynomax cat-back on my 95 that I got from summit racing. It is 2 1/2 inch piping, 2 1/2 is big enough for a Blazer, stock is 2 1/4 I believe. The only real thing i got out of it was 1-2 MPG and the sound, not much performance gain.
I looked on www.truckperformance.com and www.summitracing.com and didnt find any cat-back systems for your a non-ZR2 blazer. I agree with 20Blazer00 to check with Gibson and find out what the difference is between the kits....the ZR-2 may fit. You can also try searching google or another search engine for 2003 blazer performance exhaust.
As far as the headers, I think Edelbrock makes some, and heres a link to JBA headers for your 2003... JBA Headers
I wouldnt expect to pay less than $500 for headers, and then its pretty tight in the engine bay, so installing them will be a pain.

Sorry, forgot to include about the muffler. I have a Dynomax Super Turbo muffler. It had CFM numbers on the box and they were higher than both Flowmaster 40 series and Gibson. Gives a pretty deep tone too, deeper than other Blazers i have heard, but its not too loud inside the cab like some exhausts, (doesnt drone).
