When was the last time you did a tune up? Has the timing been verified?

Have you replaced the fuel filter?

The fan noise is likely caused by a fan clutch that has failed in the full on mode.

When the engine is cold, the fan should spin with a little resistance. If it is rigid, then the clutch is locked up.
