98 jimmy shutter/shake not moving and moving

98 jimmy shutter/shake not moving and moving
4.3,in park idles fine,rev to exactly 1,000 and it shakes,go higher.its smooth,,shakes also from a dead stop. IT has 200k,any idea what would cause this?...thanks....again!
