Default dashboard defrost vent replacement

Want to replace the plastic dashboard defrost vent in the center of the dash next to the front windshield.
The old one is cracked, seems to be a problem with a lot of the plastic stuff I have on this 25 year old Blazer.
Please see the picture, there is a projection that comes up through the vent piece.
It looks like another of the same type thing protrudes out of the right speaker cover on the dashboard at the front windshield.
Anybody know what these are? Are they different? And are they necessary?
I am primarily asking about the defroster vent because the replacement I bought doesn't have the hole in it for the protrusion.
So, anyone that has done this replacement, can the mystery protrusion be removed? Do I drill a hole in the vent?
Any advice or past experience would be appreciated. Thank you.

Hey Christine,
Thank you for your reply. Appreciate you.
Any advice on where to get an aftermarket dashboard defrost vent grill?
I cannot find anywhere that has the grill with the hole in it.

Thank you.
