Default A little help

I have an 04 blazer and i have a couple issues right now the main on is this truck starts and runs fine but under increased stress going up a hill or slow acceleration the truck revs up about 200 rpm it does it in just about every gear not really sure where to start looking. it goes back in gear just fine and doesn't affect it much but i am affraid that i could be doing some damage to the tranny. thanks for any input
