Rich exhaust

Default Rich exhaust

Had an issue with my 04 blazer. Distributor went out and I thought it was something fuel related. Replaced the pump and checked the fuel injector but it was working fine. Shortly after having it running it again the SES light came on and gave codes P0172 and P0175; fuel system too rich on both banks. Is this due to my pulling off the plenum and possibly missing a vacuum line or unseated gasket or would it be something else?

Something causing too much fuel to be delivered to both banks. Pressure regulator leaking? bad MAF sensor?

ps there are 6 fuel injectors
