On the right side of the header.... the trail blazer seems to have 3 front wheels? Or is it just me?

In that year they also put rear tires on it, which from that perspective shows between the front tires.

Yup, its just the angle the photo was taken, the 3 front wheels you are seeing is the passenger rear tire.
