Then most probably the hinge has worn out. I had to fix my Blazers door after being hit. The bushing came loose as well.

We cut the upper triangular section (with the bushing) about 1.5 cm (3/4") off the frame, fabricated a new one, welded that in place and bored a new hole by using the lower bushing as a guide for the drill. This was possible because the lower bushing was still a perfect fit for door alignment.
Then pressed the old bushing out, re-drilled the upper hole with the correct size drill and finally pulled both new bushings in.

Need to have a bolt which fits as snug as possible and a sleeve with minimal clearance to pull the bushings out and back in.

The operation has to be performed with great precision, because this fix is "non-adjustable" and if the door sags you have to cut and re-weld.

Thinking of the fix. Could work by fabricating a "weld-on" with the bushing and all and just tacking it until the door fits, then weld into place.

There are complete replacements where you drill out the welding spots on the frame and insert a plate inside the A-pillar. Then bolt the replacement on. These are adjustable.
