Default Belt slipping when cold

Hi, I have a 92 blazer which when it's started and cold the belt slips off of the ac tensioner pulley. I also have a fairly loud, consistent squeek. The belt is just over 1 year old and the squeek started shortly after that. I am looking for an opinion on what to do here, I want to try replacing the belt but it's fairly new so I'm unsure.

Any opimions/help is much appreciated, thanks.

the truck should have just one belt. So there is only one tensioner. Something isnt straight allowing the belt to come off. Most likely the tensioner or idler pulley might have failed
