Anyone ever seen this issue before?

I had that happen in my 2003 Chevy Express van and I replaced the ignition switch.
It must have had something to do with it because it never happened again.
Good luck, thats one of those problems you can't fix until it's screwing up and it doesn't screw up when you're ready to work on it.
