'99 4x4 4 door blazer sagging in the rear
what should i replace to get rid of the sag in the back?

One person that had that problem found out that when he took all the junk he had stored in the rear of his Blazer, the back end set up nice and proper. I don't suppose that would be your problem, would it?

I added a leaf from a 2nd gen 4 leaf pack, stiffened it up a bit and I also added a 2 inch shackle lift. (more like 1 though)
I have an amp under the pass seat, an amp inside the gailgate on the pass side, a t1 10 inch sub in a sealed enclosure on the pass side, and finally my spare tire carrier with bfg at on the pass side. Needless to say I sag about an inch on the pass side even after a couple added threads on the tbar bolt.

"more like 1 though" that's right, a 2" shackle will only give you 1" of lift becaues it's only pushing down half of the leafspring.
You always have the options of air bags, leveling kits, add-a-leafs etc. Depends how much you want to spend & get into it. Also what future mods you may be doing as well. Why toss $140 at a new set of leafs if later on you're going to be doing a leaf over mod?!

check out your shocks: over time (and under load) they start to give up the good fight. Gravity always wins.
Check you leaf springs. are they bowed ~ on the far end? like was mentioned, might be time for a new set of springs (jy 4 pack = $75) or an add-a-leaf
also mentioned, if you have the cash, toss in a bag. then you can set the ride height to where you like it, and bonus, when you're hauling the loaded river boat, you can pump it up a bit and not look a dog with an itchy a-hole.
